Job Summary:The Scheduler is responsible for the coordination of scheduling intermittent home health service in collaboration with the Clinical Supervisor. Enters clinical staff schedules timely and accurately into the computer daily, enters physician orders, and pertinent information to assure accurate safe staff scheduling. Communicate with staff regarding schedules.Essential Values-Based Competencies: Demonstrates values-based competencies in line with the four core values that are the foundation of all activities performed by employees in order to achieve the Mission of the St. Joseph Health System:Dignity: Demonstrates competence in communication and interpersonal relations.Excellence: Demonstrates competence in continuous improvement, continuous learning, accountability, and teamwork.Service: Demonstrates competence in customer/patient focus and adaptability.Justice: Demonstrates competence in community orientation and stewardship.Essential Functions: • Schedules staff to patient visits according to area, scope of practice, skill level (competencies), continuity of patient care and staff availability.• Enters and maintains patient schedules for all disciplines into EMR on daily basis. • Collaborates with primary care clinician and Clinical Supervisor to utilize available staff and resources in most efficient and cost-effective manner.• Manages and maintains daily schedules, assigning appropriate clinicians according to physician POC and verbal orders.• Follows regulatory guidelines for scheduling Therapy 30 day Reassessments and CHHA supervisory visits.• Maintains Start of Care log, transfer, not-taken under care, and discharges on a daily basis, continuously enters information from the log into EMR.• Notifies team members and Clinical Supervisors immediately of unsafe environments or situations, discharged, hospitalized, and expired patients or any other pertinent information.• Responds to patient, physician, and staff inquiries regarding patient schedule and takes messages in a customer service oriented manner.• Supports the Clinical Supervisors with phone coverage, scheduling needs for effective network systems.• Creates daily team lab log, ensures Labs tracked and sent to appropriate physicians.• Use of EMR application to communicate specific visit needs (i.e. NOMNC, Labs, EOO)• Annually completes Compliance Education and understands applicable policies.Additional Responsibilities: • Assists with orientation of new staff and coverage for other teams in the absence of a Scheduler.• Provides coverage for phones through main home health line as assigned. • Participates actively in continuing education, in-services, improvement processes• Performs other duties as required.Knowledge / Skills / Abilities: • Able to communicate effectively in English; bilingual is beneficial; knowledge of medical terminology is preferred.• Ability to prioritize and multi-task in a fluid fast paced environment. • Excellent organizational skills, ability to follow through with verbal and written communication.• Data entry and word- processing skills • Excellent customer service and problem-solving skills to assist Clinical Team with patient and staff problems/concerns• Good understanding of software and computer components, and can easily adapt to computer programsAge-Related Competencies: • Human development knowledge/skills: Demonstrates the knowledge and skills necessary to provide care appropriate to the age of the patients served.• Age-specific patient needs that employee is required to understand and meet: Demonstrates the ability to report age appropriate data about the patient’s status in order to identify age-specific needs and provide the care needed.• Additional requirements: Demonstrates age appropriate communication skills for the patient population served. Safety Requirements: • Is knowledgeable of, observes and complies with Home Health Network Codes of Safe Practice, safety policies and emergency procedures.Minimum Position Qualifications:Education: High School diploma or equivalentExperience: Six months of customer service experiencePreferred Position Qualifications: Experience: One year of experience in a health care setting; One year of customer service relations; experience with staffing and scheduling is preferable.Environmental Conditions: • Office environment• Knowledge of proper procedures for handling exposure to communicable diseases and blood borne pathogens.• Knowledge of the proper technique to handle exposure to hazardous material.Physical Requirements: Defined on the attached Physical Demand Analysis.
